SMP APPLICATION AND REGISTRATION (2008)
To register for an SMP trip, you must do the following:
• Complete the initial application form in the tri-fold SMP
brochure. The brochure is available in the VPC Narthex, and
online at this web page (see bottom of this page). You may also
print the Application Form (PDF file) from this web page.
Access the online SMP Application Form (PDF file) • Return the application form to the SMP box outside the VPC bookstore, or mail it directly to VPC (the address is at the top of the form). Once the initial application is received, an email will be sent to you with the full registration form. • A $100 deposit is due for all applications on or before May 4.
Scholarships:
Partial scholarships are available for those
who need financial assistance.
• Read the SMP 2008 Scholarship Policy
• Access the 2008 Scholarship Application Form which may be printed offline and submitted to VPC
Additional Steps Required:
• ADULTS: All adults MUST HAVE a Volunteer Application Form
and an SMP Information/Medical Form on file at VPC. Online
