Parent Opt-In for Media Purposes (First Child Only):  Staff and volunteers of the VPC Children’s Ministries often take pictures of various programs and activities for promotional publications within the church, including the worship bulletin, website, screens and newsletters.  (We will never identify the children by name in the photos.)

Please note:  VPC does not control the disclosure or use of photographs or video taken by participants at events that are open to parents and community members. We encourage all parents to use social media sites (i.e. Facebook, etc.) responsibly.

Buddy (Pre-K and Power-Up classes only):  Please tell us the name of ONE buddy (same grade) your child would like to be with.  Note:  Pairs of buddies need to list each other on their forms.  Please make arrangements with your buddy or children may not be placed in the same group.
