Only one Release form is required per child, even if registering for multiple activities. You will have the opportunity to complete this form online after submitting the registration below.

Staff and volunteers of the VPC Children’s Ministries often take pictures of various programs and activities for promotional publications within the church, including the worship bulletin, website, screens and newsletters. (We will never identify the children by name in the photos.)

Please note: VPC does not control the disclosure or use of photographs or video taken by participants at events that are open to parents and community members. We encourage all parents to use social media sites (i.e. Facebook, etc.) responsibly.

Acknowledgement
By clicking the "Submit" button below, I (the parent named below) grant permission for my child (named below) to participate fully in choir activities sponsored by Vienna Presbyterian Church.
I understand that this acknowledgment carries with it:

1. An authorization of any of the adult leaders to obtain necessary medical attention and/or
treatment for my son/daughter.

2. I knowingly release, absolve, indemnify, and hold harmless Vienna Presbyterian Church
from all claims that might result from any injury or death of any minor.

3. Should medical help be needed, I agree to pay either directly or through my own health and
accident insurance policy all medical or hospital costs.

4. I am fully aware that all Sunday morning worship services and some concerts are broadcast live
over the Internet by Vienna Presbyterian Church, and that when my child sings at any of these
services his/her voice will be a part of that broadcast; submission of this form indicates my
acceptance of/and agreement to this broadcast with my child’s voice in the choir.

5. I am fully aware that my child may be photographed or videotaped during Children's Choir
Activities and Concerts, and that these photos or tapes will be used to promote the VPC
Children's Choir program. Additionally, concert videos will possibly be available for sale.
