Forms and Resources

Registration for all SMPs is made by completing and submitting online registration forms and paying a $100 deposit.  The registration forms are all accessed through the Registration page.

All volunteers MUST HAVE a Volunteer Application Form on file at VPC.  This form should be printed, completed off-line, and returned to VPC; follow instructions on the form and return to VPC in a sealed envelope addressed to the attention of Human Resources.

All students MUST HAVE a Student Ministries Release Form on file at VPC.  This form should be printed, completed off-line, and returned to VPC.  The form may be mailed to Vienna Presbyterian Church, ATTN: SMP, 124 Park Street NE, Vienna, VA 22180.

Partial scholarships may be available for those who need financial assistance.  The SMP Scholarship Policy and Application Form may be accessed here.  Read the SMP Scholarship Policy, and then complete the application form and return to VPC to be considered for financial assistance.  The application may be mailed to:

The first goal of this program is to make people aware of the work being done on summer missions projects.  The second goal is to develop a team of Ministry Missions Partners who will provide us with prayer and/or financial support.  The following resources are available to help you in this part of your SMP experience.

Some additional trip-specific forms may be required, but these will be provided by trip coordinators at trip-specific meetings.
